I'm not a poetic person, so I dislike giving one of my own prayers as an example. Instead, let's focus on what a prayer before partaking of the Lord's Supper should include:

• The Lord's Supper is a memorial of Jesus' death. "For as often as you eat this bread and drink this cup, you proclaim the Lord's death till He comes" (I Corinthians 11:26). Therefore, the prayer should center around that memory. The prayer for the bread should call everyone's mind to Jesus' body, broken on our behalf (I Corinthians 11:24) and the prayer for the fruit of the vine should call everyone's mind to Jesus' shed blood that established the New Covenant (I Corinthians 11:25).
• The prayer should express thankfulness to God, the Father, for His desire to send His Son to die on our behalf. "For if when we were enemies we were reconciled to God through the death of His Son, much more, having been reconciled, we shall be saved by His life. And not only that, but we also rejoice in God through our Lord Jesus Christ, through whom we have now received the reconciliation" (Romans 5:10-11).
• The prayer can express a feeling of sorrow that it was our sins that lead to this great sacrifice on Jesus' part. "For to this you were called, because Christ also suffered for us, leaving us an example, that you should follow His steps: "Who committed no sin, nor was deceit found in His mouth"; who, when He was reviled, did not revile in return; when He suffered, He did not threaten, but committed Himself to Him who judges righteously; who Himself bore our sins in His own body on the tree, that we, having died to sins, might live for righteousness--by whose stripes you were healed" (I Peter 2:21-24).
